  I believe that's specific to Brute Force Users, What do you have set in WHM>>Server Contacts>>Contact Manager for cPhulk Alerts? 58695

  Every thing is off... just like in the image.

  That image doesn't actually show the status of any of the notifications what is the priority they're listed at? Off in this case would be disabled.
