You should talk to you hosting provider, if you are on shared server. If you have your own VPS, you should look, if you server sends spam and fix it. There is no single-solution. If you don't know how to do it - you should hire someone, who knows. Also sometimes whole network blocks might have bad reputation and in such case I would suggest changing your hosting provider.

Google and especially Microsoft have their own lists and methods to protect their users. It won't happen instantly. You need to check for error messages, that you see for email sent to Gmail, you might get some suggestions there. If your server was sending a lot of spam - it will take time, until they will release. I could suggest using third-party email relay service - basically route all outgoing mail through spam filtering gateway and block direct SMTP connections. Search for "Halon", we use that system and it helps us to protect our IP from spam sent from hacked sites or at least minimizes risk a lot. Regarding Gmail - look at Postmaster Tools " Google - there you can see status of your IP and request delisting. It is more complicated with Microsoft. We often hear complains, that emails are not delivered to Microsoft hosted domains with no reason. But still - you should start at Delist IP - Delist IP and follow instructions there.

In the first link you provide for spam-tester the biggest scoring issue is the text you're using in the email not anything specific to your server configuration - it's stated clearly that the following will cause you to be marked down:
    • This is related to the content only
    -1.985 Welcome to Pyzor"s documentation! " Pyzor 1.0 documentation) Whitelist - Pyzor)
    • This is related to the content and not the configuration
-0.001 DKIM_SIGNED Message has a DKIM or DK signature, not necessarily valid This rule is automatically applied if your email contains a DKIM signature but other positive rules will also be added if your DKIM signature is valid. See immediately below.
For the MxToolbox link you sent the only warning is:
Name Servers are on the Same Subnet
This is a common practice and not an error. If you'd like to stop receiving this error you'll need to assign one of the nameservers to a different IP address on a separate subnet. There's nothing wrong with the configuration of the mailserver from what both of these report. What needs to be investigated are as @anton_latvia stated. The provider needs to be contacted, the IP could potentially be an issue but ultimately many providers use their own proprietary rules to determine whether or not mail is or is not spam.
