Meta tag is getting added to head tag automatically

Hello, I have an odd problem. When I edit a PHP or HTML via file manager text editor and insert tag anywhere, after saving and reopening the file the following code automatically gets added within the head tag:
The problem is that all files on this account contact the above code wherever the code head is found which was not added by the user. I have 12 bare metal servers, only 1 server is like this. All of the servers are using the following blueprint:

• CloudLinux
• cPanel
• Litespeed
• Imunify360
• OSM
• Jetbackup
• CSF
