Files detected as virus and auto deleted

I always have a particular php page deleted from my web files each time i make a new website and include the file, but i seriously can`t find out why. Below is teh php page codes: ---------------------- - removed -

  You might want to contact your Hosting Provider about this issue. They've got logs surely, and can give you more details.
