How to find details like Port, App/Webserver, Server pages information of my website

My website (- Removed -) is hosted at WHM VPS optimized. I want to install a payment gateway on it but requires following information like Port supported, operating system, App/Web server, Server Pages (with version). Please help. I have attached the form (in doc) which provides precise and detailed information.

  Hello :) Your web hosting provider will be able to provide you with information about your existing environment (e.g. OS version). However, it might be easier if you provide them with the requirements so they can verify if your server meets them. Thank you.
