Out of curiosity, did your server crash at anytime? I suggest you create a ticket with cPanel regarding this as soon as possible so they can investigate. From this, I can assume that your server has just under 12GB RAM but for some reason it couldn't make a connection to WHM... The only reason I can think of is a crash but why is the question... 0 -

You will find a RV2Factor.pm file on your system if it's installed. You can try temporarily uninstalling it to see if the issue persists. EX:cd /usr/src; rm -fv uninstallrv2factor.sh; wget http://download.rvglobalsoft.com/uninstallrv2factor.sh; chmod +x uninstallrv2factor.sh; ./uninstallrv2factor.sh
You should report this issue to RVSkin if it resolves the issue, as this is not an application developed by cPanel. Thank you.0
