Malware on account issue

I can't get rid of this one anyhow. I have few joomla accounts. All up to date but somehow i can't get rid of this spammer. It generates random file names at random folders. Detected by clam. Generated files open remote connection allowing someone to quickly fill in mail q with spam. Is there anyone with the similar problem. Updated scripts, put some firewalls, IP blockers, Country blocking and changed passwords from mysql to joomla and cpanel.... Nothing. Clean one day. Tomorrow is somewhere else again. Clam register him as php.base64.v23au.185.UNOFFICIAL Thanks!
