Mailing List sending 2 confirmation Emails

I have a customer who is setting up a new mailing list. When you email into the list to subscribe it sends you 2 confirmation emails with 2 different codes. You can click to confirm the code but I can not find where or why its sending 2 email confirmations back with 2 different codes. This has been tested from multiple email addresses and all are doing this. Wanted to see if someone could help point me in the right direction where to look for this.

  Hello, Could you let us know the output from /var/log/exim_mainlog when this happens? EX:
  exigrep new-subscriber@domain.tld /var/log/exim_mainlog
