Emails sent twice or more times

One customer reported that when she sends an email message with an attachment of 2 MB of size that message is stuck in their Outlook Outgoing Mailbox and then is sent 2 or more times

  Hello, Do you notice any output to /var/log/exim_mainlog when the initial send attempt fails? Is the issue reproducible with another email or Webmail client? Thank you.

  Can you also check if the same email account is configured on multiple machines?
