In future, if you have an issue with maldet, you can report it to them via their github - they are very responsive and helpful over there.

For anyone that was interested. This was a backdoor installed by a php shell. the cleanup just didn't catch it on the first round. The files were removed, and php.ini was updated to remove a coup...

I got # grep Indexes /var/cpanel/conf/apache/local "Indexes": 0 oh and yeah its not a cached page - i can create a new subdomain and it happens.
