I had this issue and personally my fix was eliminating the "allow" line that included the admin-ajax.php AND moving that particular site to php-fpm. that seemed to clear my issue completely.

does your wordpress site have a robots.txt that contains "Allow: /wp-admin/admin-ajax.php" by chance?
