phpMyAdmin "Error: Token Mismatch"

Symptoms

When logging into phpMyAdmin, you may be presented with the following error:

"Error: Token Mismatch"

Outside of the error in the phpMyAdmin interface, no error is logged in the cPanel error log, nor in the MySQL error log.

Description 

Please check the spelling of your cPanel account's username. This error may indicate that there is a dot (.) in the username.

Workaround

To correct the issue, the username must be updated to eliminate the dot it contains. You can change a cPanel account's username via the "Modify an Account" interface in WHM.
