Segmentation faults
On a VPS we are seeing:

The machine logged altered RPM messages a few days ago - FSCK finding several errors
It logs CSF/LFD integrity errors and occasionally falsely reports services down.
Our other VPSs from the same provider have run flawlessly for 4-5 years....
Any thoughts on what is happening?

Agreed - it sounds like you're experiencing hardware issues. If you don't have good backups, now is the time to make some and be prepared to migrate your data to another machine. 0 -

It's 100% certain hardware issues with the disk. There's no question on that based on the details you posted. The only way to resolve it is to get your data migrated to a new drive. 0 -

Thank you. The host seems only capable of looking at one issue at a time. Their suggestion was turning off error messages.0 -

Oof - I'm glad you posted here then! If there really are a large amount of errors detected with fsck, just disabling errors could cause data loss when/if the drive eventually failed.0
