Hey there! You would want to check the mail logs on the system in /var/log/exim_mainlog as that would give you more details on how the messages are being sent. You may find the following page helpful in your search:
You may also want to work with that client directly as the most common way for passwords to be compromised is through key logging malware on the client's machine.
If you'd like to temporarily suspend the email account from being able to send messages while you investigate the issue, we have an option here for that work:

You would then use https://api.docs.cpanel.net/openapi/whm/operation/unsuspend_outgoing_email/ to unsuspend the user.

Thank you very much for your answer. We already contacted our user and he deleted problematic email accounts from which many spams were sent since he did not even create them himself.
