Mail Delivery Reports not showing all email
I've taken over management of a server at v86.0.40 (don't yell at me, please), but Mail Delivery Reports are now showing me all of the detected spam in the Exim log.
What setting(s) needs to be corrected so that I see all email passing through our server?

Hey there! This sounds like old behavior mentioned in case https://support.cpanel.net/hc/en-us/articles/4402160366231-Email-delivery-attempts-are-missing-in-WHM-Mail-Delivery-Reports
I'm not seeing any recent reports of this behavior, but if you can update that system to a more modern version (I know that may not be possible) and you're still seeing this, we'd like to see a ticket if possible so we could confirm the behavior.
Note: I deleted the previous comments here as they were spam bots.

I probably should have been clearer that I am not seeing all "received" (not sent) mail that is detected as spam. Thanks, though.

If you manually check the exim_mainlog file, do you see the mail there? If so, it would seem like that case is your issue.
