Hey there!  It sounds like there are no quotas configured in the operating system itself.  What happens when you run "/scripts/initquotas" command on the command line?

    I'm facing the same error:

    [root@s1 ~]# /scripts/initquotas

    /dev/vda2 (already configured quotas = 1).
    Updating Quota Files......

Quotas have been enabled and updated.

I still get the same error

Mthokozisi - I have the team looking into this now and I'll post an update as soon as I have one to share.

At this time it seems a reboot is fixing this issue.  I know that is disruptive, but you could try a reboot at a time when that is convenient for you and see if that resolves the issue?

Thank you for your response, I tried rebooting the server but still the same error. It seems like I cannot create new accounts.

    Someone suggested I check this:

    vi /etc/fstab

    response:

    /dev/vda2       /       ext4    defaults,usrquota,grpquota        1       1
    /usr/tmpDSK             /tmp                    ext4    defaults,noauto        0 0

It would be best to contact your hosting provider about this issue as that all looks to be configured correctly.
