    Hey there!  If you can't find the mail ID in the exim_mainlog file that would indicate that something other than Exim is creating those messages.  While I hate to say it, it sounds like something on the account or system may be compromised.

    I would start by creating a ticket with your provider (or us, if your license is purchased through cPanel directly) so they could look into possible account compromises.

    I would like to add that this email account is shared on 3 different computers.
    It seems that Outlook (IMAP) sync this account in both ways, so if I delete this spam email, it stays in Outlook spam folder on other PC which might be offline. Once online it upload thousands copies of that email.

    Not sure if I should still see the ID message in the exim_mainlog file when Outlook syncs the spam folder from the PC to the email server?

    No you should not and would not see anything in exim_mainlog when IMAP syncs.  Sounds like you figured out your issue.   Deelete the emails from the Outlook spam folder on the other PC so that it will sync those changes back to the server.

    That would explain it, and a two-way sync wouldn't trigger another delivery report in the exim_mainlog.
