Hey there! Can you let me know what actions led to this error being generated? Is this possibly related to WHMCS?

I did a search through old log files and I found another instance of it, but only one! That instance happened on a totally different server of ours that was running CL6 back in May this year. In June we had a new CL8 server provisioned and all the accounts were migrated over to it using Live Transfer. There has been no server administration carried out beyond the restoration of a client's deleted account using R1Soft two days prior.

Well that's interesting :D Is there something broken, or did you happen to see this in a log file somewhere? I don't really have any good info on this one on my end as I only see it coming up one other time in our ticket history as well, and that instance was related to a bad custom database query. In general, I don't think I'd expect root to have a dbmap, actually. If it's just a one-off log entry, I'd ignore it. If you can make the error happen with a specific server, we'd probably want to see a ticket on this.
